ACORD 24 Type of Coverage and Covered Property


ACORD 24 Certificate of Property Insurance Instructions

TYPE OF INSURANCE

PROPERTY - Check the check box to the left of the CAUSES OF LOSS text if that is the coverage provided.

DEDUCTIBLES - enter the dollar amount or percentage of the deductible in the space to the right of the CAUSE OF LOSS if there is a deductible. Enter both the building and the contents (personal property) deductibles separately even if they are the same.

CAUSES OF LOSS

BASIC
BROAD
SPECIAL

These three items are mutually exclusive. Only check one.

EARTHQUAKE
WIND
FLOOD

These are all stand alone coverages not included in the three coverages listed above.

BLANK ROWS - these two rows are for you to list any other covered causes of loss included in this policy.


INLAND MARINE

CAUSES OF LOSS - There are only two check boxes available. The first should be checked if this is a NAMED PERILS policy. Many Inland Marine policies are nonstandard manuscript forms. If that describes this policy check the second check box and enter a text description to the right of the box.


CRIME

Crime gives you a check box but no Cause of Loss choices. If you are providing Crime coverage you will have to enter a text description of the policy type.


BOILER MACHINERY / EQUIPMENT BREAKDOWN

If you are providing either of these coverages I am taking a moment here to congratulate you. Only a small percentage of insurance agents understand these coverages enough to offer them. That is a shame because they are very valuable protections. Did you know Equipment Breakdown coverage can cover a copy machine, a desktop computer, the coffee maker, even a typewriter if anyone still has one?

All you need to do here is check the check box if you are providing this coverage.


POLICY NUMBER

ACORD 24 Certtificate of Property Insurance Instructions

For each Type of Insurance your have indicated you are providing in the Type of Insurance column to the left of this column you should enter the full policy number.

In the section next to INLAND MARINE you are also asked to enter a text description of the TYPE OF POLICY. This would be something along the lines of Jewelry, Fine Arts, Furs, Cameras, Musical Instruments, Guns, etc.


POLICY EFFECTIVE AND EXPIRATION DATE

For each policy listed on the document enter the policy effective and expiration date.

Enter them in this format...

01/01/2015 instead of January 1, 2015.

It is a computer system friendly format.

How to Complete the ACORD 24 Certificate of Property Insurance

CERTIFICATE NUMBER - if your agency assigns certificate numbers to issued certificates, this is where you should enter that number. Tracking certificates this way is a good idea if you issue a lot of certificates for a specific insured.

REVISION NUMBER - if your agency does assign certificate numbers and they have to change anything on a certificate already assigned a certificate number, this is where you would enter a revision number. Normally the first revision would be number 1. This means this original certificate would have been 0 and no revision number would have been entered.

ACORD 24 Location of Premies

LOCATION OF PREMISES / DESCRIPTION OF PROPERTY

Enter the physical location of the property this certificate covers. Normally this is the physical address. But sometimes the physical address is for several related buildings - house, guest house, barn, garage, garden shed, boat house, tractor shed, etc. In these cases you will need to include a more detailed description.

If you need more room for the description you should use the ACORD 101 Additional Remarks form.


ACORD 24 Limits May Be Reduced Statement

ALL CAPS WORDING

The text section immediately below the Location of Premises certifies the policy has been issued. It also states clearly that this certificate, or any other document not the actual policy cannot change the policy. Finally it makes clear that the coverage limit shown on this document may be reduced upon payment of claims.

INSR LTR

This stands for Insured Letter.

To the right of the Insured name and address section near the top of this page you entered the insurer's names and NAIC numbers. Enter the INSURER LETTER - A, B, C, D, E or F - for the insurer which is providing the coverage in the rows corresponding with this column.

If the coverage described in any column is not provided by any carrier, do not enter an Insured Letter.

PRODUCER INFORMATION - enter your agency's name and address.

ACORD 24 Insured Information

INSURED - enter the insured's name and address.

INSURED CONTACT INFO

CONTACT NAME - the name of the contact person for this insured.

PHONE (AC, No, Ext) - The contact person's phone number. A/C stands for area code. No stands for number. Ext stands for extension.

FAX (A/C, No) - The contact person's FAX number. A/C stands for area code. No stands for number.

E-MAIL ADDRESS - Enter the contact person's email address.

PRODUCER CUSTOMER ID - If your agency management system uses Customer and Producer numbers for accounting purposes - especially for commission payment to you - enter those numbers here.


INSURER(s) AFFORDING COVERAGE

ACORD 24 Certificate of Property Insurance Carrier Info
INSURER A: INSURER B etc, - List the name of the insurance carrier providing coverage. Use the actual masthead name - not the group name. For instance, The Travelers is a holding company which owns several insurance carriers - mastheads. One of those carriers owned by The Travelers is Charter Oak Insurance Company. You should enter Charter Oak instead of The Travelers.

NAIC# - NAIC stands for the National Association of Insurance Commissioners.

ACORD 30 Certificate of Garage Insurance Instructions


COMMON COLUMNS

The following columns are the same for each of the following sections...

INS LTR - This means Insurer Letter. Look up in the second section down on the far right hand side of this form. You entered the carrier's names next to a corresponding letter - INSURER A, INSURER B, etc. Type in the letter which corresponds to the Insurer providing this coverage and enter that letter in this space.

ADDL INSD

Place a Y in this column if there is an Additional Insured. Remember, additional insureds have to be added to the policy by endorsement. Checking this box on the certificate does not modify the policy.

SUBR WVD

Place a Y in this column if Subrogation has been WAIVED by endorsement under this policy. Remember this also has to be done by endorsement to the actual policy.

POLICY NUMBER

Enter the Policy Number for the policy providing the coverage indicated to the left on this form.

POLICY EFF (MM/DD/YYYY)

Enter the policy effective date - the day coverage begins.

POLICY EXP DATE (MM/DD/YYYY)

Enter the policy expiration date - the day coverage ends.



COMMERCIAL GENERAL LIABILITY


TYPE OF INSURANCE

Under Commercial General Liability your have six check boxes. Put a check or an "X" in the boxes which apply.

COMMERCIAL GENERAL LIABILITY - the first check box indicates there is Commercial General Liability coverage. If this is not checked you will not be checking any of the other check boxes in this section.

CLAIMS-MADE / OCCURENCE - We have a good post on the difference between Claims Made and Occurrence at this link HERE. Check the appropriate check box. These are mutually exclusive. You can only be one or the other so only check one box.

TWO BLANK DESCRIPTIONS - the next two check boxes are available for you to enter your own coverage description. If you check a box, be sure to enter a text description of the coverage to the right of the checked box.

GEN'L AGGREGATE LIMIT APPLIES PER:

This is the maximum amount the insurer will pay. Your choices are...

POLICY - if the maximum is based on the policy period

PROJECT - if there is a maximum that will be paid on any one project.

LOC - if there is a maximum which will be for any specific location

OTHER - enter you own text description

LIMITS

Enter the dollar amount of this policy's coverage next to the provided coverage. If the coverage is not provided under this policy, leave the space blank.

UMBRELLA / EXCESS LIABILITY

UMBRELLA or EXCESS - Check the appropriate check box to indicate whether this policy is and UMBRELLA policy or an EXCESS policy.

An Umbrella policy provides liability coverage after the underlying, or primary, liability policy has been paid out. Umbrella policies have a significant retention or deductible amount the insured must pay after the primary insurance policy pays. The umbrella policy has its own terms and conditions and may provide coverage the primary policy does not provide or restrict coverage to not cover some losses the primary policy does cover.

An Excess policy can be an Excess Follow policy. This is where the coverage, terms and conditions track the primary policy.

OCCUR or CLAIMS MADE - check the appropriate check box. We have a good post on the difference between Occurrence and Claims Made coverage HERE.

DED - stands for Deductible.

Enter a check mark in either the Deductible or Retention box.

Deductibles and Retention - self insured retention - are not the same. They differ in the responsibilities of the carrier when a loss occurs, defense costs, collateral requirements, limits erosion and certificates of insurance details.

Generally speaking I have been told the deductible is more favorable to the insured. My Daddy always told me for a variety of reasons that it is always best in the event of a loss to have all your policies with the same carrier.

ENTER the AMOUNT of the Deductible r the Retention in the blank space to the right of the $ sign.

LIMITS

Enter the dollar amount of the limit for the coverage below if that coverage is provided by the policy listed to the left of this space.

EACH OCCURRENCE

AGGREGATE

BLANK - enter your own coverage and then enter the amount.


WORKERS COMPENSATION AND EMPLOYERS' LIABILITY

Put an "Y" for yes or an "N" for no in the check box nest to the question "ANY PROPRIETOR/PARTNER/EXECUTIVE OFFICER/MEMBER EXCLUDED?"

If anyone is excluded, describe in the REMARKS section on this form.

LIMITS

Under Limits, to the best of my knowledge and research, all states have limits per Statute, so you would always check this. Some states allow the business to purchase limits in excess of the statutory limits. You need to ask your underwriter if that is available in the state in which you are providing coverage.

Under limits there are three lines which start with E.L This stands for Employers Liability. This provides coverage that protects the company in the event that an employee alleges that the employer's negligence or failure to provide a safe work place was the cause of the employees's illness or injury. refer to your actual policy for these limits.

How to Complete the ACORD 30 Certificate of Garage Insurance
In the line to the right of the title "COVERAGES" are three items you may or may not need to fill in.

PROD / CUSTOMER ID - This would be the identification number from your agency management system

CERTIFICATE # - If your agency tracks certificates by number, enter this certificates tracking number here.

REVISION # - If your agency tracks certificates by number and this is a revision of an existing certificate, enter the revision number here.

COVERAGE MAY BE REDUCED WORDING

Read the box of ALL CAPS words under the "COVERAGES" Title. As an insurance professional you need to be aware of this. The important part is the phrase "LIMITS SHOWN MAY HAVE BEEN REDUCED BY PAID CLAIMS"

GARAGE LIABILITY

Check one of the boxes to the left of COMP / OTC (comprehensive  / other than collision), or SPECIFIED PERILS. if either of those coverages are being provided by this policy. These are mutually exclusive.

Check the check box to the left of COLLISION if that coverage is being provided by this policy.

To the right of these coverage text description, if these coverages are provided by this policy, enter the location number.

To the right of the location numbers enter the limits of coverage.

This form cannot change the terms and conditions of the policy

If the Certificate Holder is also an Additional Insured you HAVE TO ADD THEM TO THE POLICY BY ENDORSEMENT - see rule #1 above.

PRODUCER - Enter the agency name and address

CONTACT NAME - enter the agent or CSR's name

PHONE (A/C, No, Ext) - the the contact person's phone number. A/C stands for area code, No does not mean no it means number. Ext means phone extension if there is one.

Example - 919-384-0514 Ext 21

FAX (A/C. No) - enter contact person's FAX number for those few agents who still use Fax machines.

E-MAIL ADDRESS - enter contact person's business email address. You should never use a personal email address for business. It leaves you personally open to various legal exposures.

INSURED - enter the insured's name and address exactly as you want or need it to appear on the policy.

INSURER(s) AFFORDING COVERAGE
INSURER A, B, C, D, E and F

Enter the name of the insurance company which will be on the policy declaration page. This is not the holding company group name - ex: The Travelers - but the specific company in the group - ex: Oak River Insurance.

NAIC#

NAIC stands for the National Association of Insurance Commissioners.

ACORD 29 Evidence of Flood Insurance Remarks, Cancellation and Additional Interest Section

REMARKS

The Remarks section of the ACORD 29 is very small. Of course about the only thing you might need it for is to expand on the Description and Location of the property / properties.

CANCELLATION

There is nothing for you to enter in the cancellation section. But you do need to know what it says. Basically many insured's would ask the agent to use Evidence of Insurance forms to commit to a cancellation notice beyond what the policy itself grants. This can no longer be done through an Evidence of Insurance form.

ADDITIONAL INTEREST

NAME AND ADDRESS - Enter the name and address of this insured exactly as it is shown on the policy.

LOAN NUMBER - Enter the loan number or account number for the loan this additional interest has with this insured.

TYPE OF INTEREST

Check the check box next to the appropriate description of this additional interest legal relationship with this insured. These are mutually exclusive. Check only one. The options are...

MORTGAGE
LOSS PAYEE
UNIT OWNERS MORTGAGEE ( Does not imply interest)
ADDITIONAL INSURED
BLANK next to check box. Enter a text description of the interest.

NAMED ON POLICY (Check all that apply)

Check the appropriate check box next to each  policy type for which policy this Additional applies. You can check them all. The options are...

Instructions for the ACORD 29 Evidence of Flood Insurance

ACORD 29 Policy Information Section

The ACORD 29 Evidence of Flood Insurance form has three policy sections...

  1. Primary Policy
  2. Excess Policy #1
  3. Excess Policy #2



You will complete the two excess policy information sections exactly the same as the primary section - if you have excess policies -  so I will not cover them separately. There are a few additional questions in the EXCESS POLICY sections and I will discuss those.


COVERAGE  / RISK INFORMATION


NOTE: Look to the far right hand side of the form on the line with the title of this section "COVERAGE / RISK INFORMATION" and you will see this...

"*12:01 A.M. LOCAL TIME AT THE INSURED PROPERTY LOCATION"

This, of course, is the standard time for all property coverage effective and expiration dates unless you enter another time.

PRIMARY POLICY


POLICY NUMBER - the policy number for this issued policy. If the policy is bound but has not yet had a policy number assigned I suggest you enter the word "BOUND"

EFFECTIVE DATE - the date this policy's coverage will begin. See note on effective time above. 

EXPIRATION DATE - the date this policy's coverage will end, cease or stop. See note on expiration date time above.

INS LTR - this means Insurer Letter. Look up in the second section down on the far right hand side of this form. You entered the carrier's names next to a corresponding letter - INSURER A, INSURER B, etc.

Enter this corresponding letter.

COVERAGE - this is a column heading for the two lines below. the first line is BUILDING. The second line is CONTENTS. 

DEDUCTIBLE - this is a column heading for the two lines below. Enter the appropriate building and contents deductibles. If you do not have building or contents coverage, leave the space blank.

TOTAL AMOUNT OF INSURANCE - Enter the Total limit of insurance under this policy for building and contents. If you do not have both of those coverages on this policy, leave the limits blank  as appropriate.

MARKET - check the appropriate box for the program this policy falls under.

The Excess Policy sections do have one check box the primary policy section does not have.

INDICATES EXCESS POLICY IS A "FOLLOWING FORM" POLICY TYPE

Basically a "Following Form" excess policy states that it follows all the terms and conditions of the primary policy underneath it.

How to Complete the ACORD 29 Evidence of Flood Insurance

ACORD 29 Agency, Insured and Carrier Information



The ACORD 29 Evidence of Flood Insurance starts with the date field in the top right hand corner of the form. This should be the date you are completing this form.

The next item to note is the BOLD PRINT in the top box on the form. This form is not a part of the policy. As such it cannot be used to modify any portion of the policy or give any rights to any party not provided by the actual policy.



INSURANCE AGENT / PRODUCER

Enter the Agency name and address. You should enter your name - the producer - in the form of ATTN: John Agent,


CONTACT NAME

The name of the agent or CSR this insured will work with.

PHONE (A/C, No, EXT)

The phone number for the contact person. A/C stands for area code. No stands for number. EXT means the contacts phone extension if they have one.

FAX (A/C, No)

The FAX number for the contact person. A/C stands for area code. No stands for number.

E-MAIL ADDRESS

The email address for the contact person.

PRODUCER CUSTOMER ID #

If your agency still assigns customer ID numbers, enter that here.

NAMED INSURED AND ADDRESS

Enter the insured's name exactly as it appears on the policy dec page.

ACORD 28 CANCELLATION AND ADDITIONAL INTEREST SECTIONS



CANCELLATION

The cancellation section is simple. Thee is nothing to complete. But you should be aware of what it says, which I quote below...

"Should any of the above described policies be cancelled before the expiration date thereof, notice will be delivered in accordance with the policy provisions."


ADDITIONAL INTEREST

Check the appropriate box to indicate if the additional interest is a MORTGAGEE, LENDERS LOSS PAYABLE, CONTRACT OF SALE or other.

If Other enter a description.

NAME AND ADDRESS

Enter the name and mailing address of the additional interest. If there is more than one additional interest you should issue a separate ACORD 28 for each.

LENDER SERVICING AGENT NAME AND ADDRESS

Enter the name and mailing address.

AUTHORIZED REPRESENTATIVE

Your signature if you are a licensed agent for the insurance company providing coverage on this form and if your contract with that insurance company grants you authority to sign.

ACORD 28 Evidence - Coverage Information

How to Complete the Coverage Information in the ACORD 28


There are many different coverages in a commercial property policy. I will try and give you a brief explanation of each with links to more detailed information.


PROPERTY INFORMATION


To the right of the bold print "PROPERTY INFORMATION" are two check boxes. One for BUILDING and one for BUSINESS PERSONAL PROPERTY.

Check either or both.

LOCATION / DESCRIPTION

Enter the physical address of the location or locations for this policy. If you need additional information to describe the location beyond the address enter that also.

If you need more space to list locations use the REMARKS section on the second page of this form.

COVERAGE INFORMATION


Just to the right of the title "COVERAGE INFORMATION" you will see Perils Insured.

PERILS INSURED - check the appropriate box for the type of coverage form you are using for this policy - Basic, Broad or Special. these are mutually exclusive. Just check one.

NOTE: For all the following items you will check one box in the center three columns. The columns are titled "YES", "NO" and "N/A". N/A stands for not applicable - which in turn means this coverage does not apply to this insured.

BUSINESS INCOME / RENTAL VALUE

The IRMI website begins its definition of Business Income insurance with "Commercial property insurance covering loss of income suffered by a business when damage to its premises by a covered cause of loss causes a slowdown or suspension of its operations. "

The same site defines Rental Value coverage as "Time element property insurance that pays for loss of rental income when a building that is rented out to others has been damaged by a covered cause of loss. Coverage is also provided for the fair rental value of the portion of the premises occupied by the insured. Rental value insurance can be provided by a business income coverage form or a specialized rents or rental value coverage form."

If either of these coverages are being included you need to enter the limit and enter the number of months of coverage.

BLANKET COVERAGE - Blanket coverage is using a single limit of insurance to cover more than one item, more than one location or more than one category of property coverage. An example would be one limit for both buildings and contents coverage.

If coverage is on a Blanket basis you need to indicate the limit of the property listed in the Location/Description section.

TERRORISM COVERAGE  - This is defined in large part by the TRIA (Terrorism Risk Insurance Act). Here is a link to a good discussion of the 2007 re-authorization of that act...

If Terrorism coverage is included you need to attach the Disclosure Notice or the policy declaration page to this ACORD 28 form.

Under this you need to check the appropriate box for the two sub questions..

IS THERE A TERRORISM-SPECIFIC EXCLUSION?

IS DOMESTIC TERRORISM EXCLUDED


LIMITED FUNGUS COVERAGE - There are a large number of search results for this coverage relating to the various FAIR Plans around the country. I strongly encourage you to become familiar with your state's forms.

Here is a LINK to the ISO BOP policy form and that definition.

If Limited Fungus coverage is included you need to enter the limit of this coverage and the deductible amount.


REPLACEMENT COST - Replacement cost coverage is normally defined as the cost to replace or rebuild the property with same quality and materials without depreciation.

AGREED VALUE - IRMI defines this as "A commercial property insurance provision that suspends the coinsurance clause until a specified expiration date. Insurers usually require a statement of property values signed by the insured as a condition of activating or including an agreed value provision in a commercial property policy."

COINSURANCE - Check whether the co-insurance clause is in this policy. If co-insurance does apply to this policy enter the percentage.

Co-insurance percentage is the percentage of the value of the Subject of Insurance - listed in the Location/Description field - which is being insured. A normal amount is 80%. If the amount of insurance falls below this percentage amount your insured will be required to participate in paying any claims.

EQUIPMENT BREAKDOWN (If Applicable) - this coverage is for loss as a result of mechanical or electrical breakdown of more or less any kind of equipment. That means not just a bulldozer but also a desktop computer or a copy machine. Coverage often also includes loss of business income.

If this coverage applies you need to enter the limit of coverage and the amount of the deductible.

ORDINANCE OR LAW - Older buildings are normally Grandfathered in, meaning as the building codes are updated, existing buildings do not have to re-wire, re-plumb, etc to meet the new building codes. However, when there is damage to the building that has to be repaired - for instance a fire - the repair work must meet the existing new building code requirements.

The options on the ACORD 28 form are...


  • Coverage for loss to undamaged portion of bldg
  • Demolition Costs
  • Incr. Cost of Construction - INCR stand for increased


If you check yes for any of these coverages you need to enter the limit of coverage and the deductible amount.

EARTH MOVEMENT (If Applicable) -Earth movement insurance is not Earthquake insurance. Instead it covers many smaller events which can still cause damage to  a structure.

If this coverage is included you need to enter the limits and the deductible amount.

FLOOD (If Applicable) - Flood is normally covered under a separate flood insurance policy. If this coverage is included you need to enter the limits and the deductible amount.

FEMA defines a flood as...

A general and temporary condition of partial or complete inundation of two or more acres of normally dry land area or of two or more properties (at least one of which is the policyholder's property) from:
--Overflow of inland or tidal waters; or
--Unusual and rapid accumulation or runoff of surface waters from any source; or
--Mudflow;or

Collapse or subsidence of land along the shore of a lake or similar body of water as a result of erosion or undermining caused by waves or currents of water exceeding anticipated cyclical levels that result in a flood as defined above.

Your specific flood policy may not use these same definitions. You need to read your policy.

WIND / HAIL INCL - INCL stands for included. This covers damage from wind and hail. You need to check the YES or NO box to indicate if this coverage is subject to different conditions that the other coverages shown on this form. That is not unusual. Most often the deductible is applied as a percentage.

If this coverage is included you need to enter the limits and the deductible amount.

NAMED STORM INCL - Insurance for Wind and Hail can be purchased for specific named storms. They are also normally subject to different provisions.

If this coverage is included you need to enter the limits and the deductible amount.

PERMISSION TO WAIVE SUBROGATION IN FAVOR OF MORTGAGE HOLDER PRIOR TO LOSS

The Waiver of Subrogation prohibits the insurer from attempting to seek restitution from a third party who causes any kind of loss to the insured. This type of arrangement is allowable under certain circumstances where the insured could be held liable for a claim that is paid.

The ACORD 28 is for commercial property insurance what the ACORD 25 is for liability insurance.

Be sure to read the bold print in the top box of this form. The Evidence of Commercial Property Insurance is informational only. It does not modify the policy. It does not give any rights or additional interest to any party.

In this post we will cover the inputs you need for the sections highlighted below.


DATE

Enter the date you are completing this form.





AGENCY INFO

PRODUCER NAME, CONTACT PERSON AND ADDRESS - enter Agency name as the Producer name. Enter your name as the contact person.

PHONE (A/C No. EXT) - A/C means area code. No. means phone number other than the area code. EXT means your personal extension.

Example:   919-384-0514 ext 17

FAX (A/C, No.) A/C means area code. No. means phone number other than the area code.

E-MAIL ADDRESS - your work email address. You should never use your personal email address for work related communications. This exposes you to many legal issues to which you do not ever want or need to be exposed.

CODE - This is your agency code for the specific carrier to which you are sending this application. This can be left blank often.

SUB CODE - This is your specific producer code for the carrier to whom you are sending this application. The reason for the agency code and the sub code is for the carrier to present commission reports broken down this way. Your carrier may or may not have that capability.

AGENCY CUSTOMER ID# - If you have an old agency management system you may still need to track that systems customer ID number. You should get a newer system that manages data better.


INSURED INFO

NAMED INSURED AND ADDRESS - Enter the name the way the lending institution will need to see it. Probably this means matching the way it is listed on any loan documents.

ADDITIONAL NAMED INSURED(S) - Name only, not address.


COMPANY AND POLICY INFO

COMPANY NAME AND ADDRESS - enter the company masthead name, not necessarily the same as the company group name. Example: The Travelers is a group of companies - a holding company. Oak River Insurance Co is a masthead name - an actual insurance company who will be shown as the insurer on the policy.

NAIC NO. - NAIC stands for the National Association of Insurance Commissioners.

POLICY TYPE - commercial property, builders risk, etc.

LOAN NUMBER - the account number or loan number the lending institution uses to identify this customer.

POLICY NUMBER - If the policy has been issued or had the number assigned you will enter that here. If not, leave this blank.

EFFECTIVE DATE - the day, month and year this policy's coverage is to begin.

EXPIRATION DATE - the day, month and year this policy's coverage ends. For an annual policy this would be the same date the following year.

Example - effective date 10/01/2014  would have an expiration date of 10/01/2015.

CONTINUED UNTIL TERMINATED IF CHECKED - check this if the policy has been issued on a conditional basis.

THIS REPLACES PRIOR EVIDENCE DATED - Enter the date of any Evidence document which may have already have been issued.

ACORD 193 Fraud Warning and Signatures

The fraud section is the top two thirds of the ACORD 193 second page.


This is a group of Fraud Warnings. Before you sign, and especially before your customer signs this document, you should review this section. That is just good basic business practice.


Signatures

At the very bottom of the 2nd page is a place for the Applicant's Signature and next to it a space to have the insured date their signature.

Above the Applicant's Signature is the place for you - the agent - to sign. Just to the right of your signature you should print your name.

To the far left in your signature row is a box for your State Producer License Number.

Finally there is a box for your National Producer Number if your agency has one.
